Many serious accidents have occurred as a result of an equipment started running when it was supposed to shut off for maintenance or repairs. These incidents are typically caused by energy that was not released properly. Lock-out tag-out (LOTO) systems help prevent these accidents by making sure that the machines remain off until the person who is authorized releases them.
The LOTO system is comprised of a number of locks, tags and other devices that are used to isolate, secure, or block a device from an electric source. All "affected", "authorized" and "qualified" personnel should be trained on the use of lockout and tagout devices. These devices include but not restricted to chains, locks padslocks, wedges adapter pins and self-locking fasteners. Each device is uniquely identified and labeled, including the date and time that it was put in use as well as the name and title of the authorized person and the reason for the lockout or tagout (repairs maintenance, repairs or repairs, etc.). It must also be clearly stated that these devices can't be removed until a complete LOTO procedure is followed.
